Scheduling an Appointment
To schedule an appointment with one of our board-certified physicians or nurse practitioners, please contact the office you would like to visit during regular business hours. Be sure to tell our staff the nature of your health concern so we may allow ample time for your appointment.
If you would like to schedule a routine exam, such as a physical, please be sure to call several months in advance so that we can find a convenient time for you. If you are unable to keep your appointment for any reason, please contact us as soon as possible.
Prior to your first visit, we will mail you some health history and insurance forms to complete. Please bring these with you to your appointment, along with your insurance card. You also will need to complete a personal and family health history before your first examination and consultation. By being prepared for your first visit, we can make the best use of your appointment time to thoroughly evaluate your health care needs.
